Neurologist, Primary: Get a Nerve Conduction Velocity test to check your nerves. Also get a diabetic evaluation from your primary care doctor

Hot heels: compression of nerve roots L4,5 and S,1,2 can cause any where from burning, tingling, itching sensation in the feet/heels. that is why it is not hot to the touch.
probably happens at night.
Apply ice to low back to see if symptoms subside

May need a doctor: If a person gets small brown spots on the bottom of his feet, sometimes they are warts. Other times they are a fungus. If fungus, twice a day over-the-counter lotrimin (clotrimazole) af for 2-3 weeks may solve the problem. If warts, a primary care doctor or dermatologist will likely have to freeze them off. A visit to the doctor can help get a diagnosis, and see if the spots aren't something else.

Need for a diagnosis: Yes it could be a superficial fungus (eg athlete's feet), and perhaps it was not treated long enough with the right cream and powder; these critters grow in dark, moist corners of the body. It may be another problem, some infections cause itchy feet. Is it the whole foot, does it correspond with your shoeware (contact dermatitis). May need gp or dermatologist to diagnose and fully treat the prob.

No: Kidney stones will produce pain, bilateral feet swelling can be a number of problems including congestive heart fair, vascular insufficiency, chronic renal failure, etc. But chronic swelling needs to be examined and worked up further by your doc which will include lab testing. Hope this helps!
