A low salt diet (2 grams per day or less) can help reduce swelling. Others options for swelling can include compression stockings or a stronger diuretic.

No: If your fluid retention is worse, it can be due to any number of reasons. Where is your fluid retaining? Seeing that you're 34, you can retain fluid during your cycle. Diets high in sodium content can do it. If someone is very obese, it can prevent fluid from returning properly to the heart. But I find it odd that you have been on hydrochlorothiazide for 4 years, thus starting at the age of 30.

No it should not: HCTZ (hydrochlorothiazide) should not cause fluid retention. However if you are retaining more fluid despite being on it, then your doctor need to access the reason for your initial fluid retention in the first place and then decide if you need other diuretic. There are many reasons why people retain fluids such heart failure, liver disease etc. And your doctor need to figure that out..
