Lots of things: Too many to go over in such a brief response forum. Poor sleep hygiene likely accounts for a large part of sleep onset problems whereas medications, physical/mental issues, parasomnias such as nightmares, restless legs, Parkinson's disease, aging can cause sleep maintenance issues. A person needs a full and dedicated evaluation by someone who is well versed in all these things.

Many causes: There are many reasons for problems with both sleep onset and maintenance. Illnesses such as alcohol abuse, depression, bipolar disorder and anxiety can be the cause. Metabolic illnesses such as hyperthyroidism are also a possibilities. In addition stimulants such as cocaine, amphetamines, decongestants, caffeine and nicotine can contribute to this.
