Strangely enough: Patients with ankylosing spondylitis have a very normal appearing MRI if they don't have a fracture. Ankylosing spondylitis causes all the ligaments anterior and posterior to calcify so it is easy to see on xray but on MRI since these structures are dark and hard to see it will be hard to see that they are thickened.
