See a doctor: The back and chest pain with breathing suggest inflammation of ribs and possibly the lining of the lungs. I would have expected these symptoms sooner after the accident, and if they are new it's possible something more serious is afoot. Clots in the lung are a concern, especially if there is shortness of breath. Please see a doctor as soon as possible.
