Urgent: While I only have the small amount of information you provided, but this sounds like an emergency. It the pain is so severe that you can't sleep and your are having difficulty breathing you need an urgent in person evaluation. This could be related to very concerning heart and/or lung pathology, especially in the setting of a recent surgery. Please seek help.

Could be: a pinched nerve. These are common especially when we sleep the wrong way. Chest pain should always be concerning and get checked out, but we see it alot with the pinched nerves which can start in the neck. When muscles / nerves are effecting in the chest cavity, often times, there is the pain during breathing.
