Testicular pain&swell when sitting, but not stand or lying down. the perineum is also sore could this be pudendal neuralgia? if so how is it treated?

Testicular swelling: Unlikely to be pudendal neuralgia. If the swelling goes away while standing, there is most likely some venous obstruction. Please see a urologist for proper evaluation. Even a general surgeon can advise you properly after examination. Good luck.

Testicular pain: most likely you have Epididymitis and or Prostatitis. you will need to contact health care providers and need to be treated.
