Urology consult: Please get examined by an expert ( urologist). You may need not only an exam, but also cystoscopy and visualization of the urethra.. I would also get tested for exposure to HSV2 ( herpes genitalis).

Some approaches: There are nerve pain suppressant drugs, such as gabapentin or Lyrica, (pregabalin) but there might be some value in seeing a physical therapist, one who is skilled in pelvic floor approaches.
