Could be either: A painful, swollen big toe joint that occurs from time to time is likely to be gout or other type of arthritis. A permanent bump that is more painful after wearing shoes all day is more likely to be a bunion. By your description, bunion is more likely. See an orthopedic foot doctor or podiatrist for an accurate diagnosis and treatment plan.
