Difficult to say: Unfortunately there are multiple causes for painful lesion on the genitals. Without having a better understanding of what it appears like and what may have been going on prior to you noticing it, it's difficult to provide you with good advice. The best I can suggest is to see your doctor so that appropriate treatment can be determined, don't wait too long as it could be contagious or spread.
