The description is based on the radiologists experience, and is based on the appearance of the appendix and the tissue around it. Only a surgical exploration can tell for sure what is going on. Appendicitis usually means infection, while many things can make an appendix appear prominant.

Changes: In an inflamed appendix there are changes involving the wall and the immediate surroundings as the inflammation propagates through while a prominent appendix is just a large appendix with no changes involving its walls.
