Enlarged thyroid: You need to find out why and what is causing your thyroid to get larger. A thyroid that is causing you to choke is very concerning. Make an appointment as soon as possible to see a doctor. This could be caused by lack of iodine to cancer and lots of things in-between. Do not wait, get it evaluated.

Endocrine: Please see an Endocrine doctor, depending on what kind of lesion your thyroid have the treatment options are different, but if I take a guess you might have Multinodular Goiter which could be treated with Radioactive Iodine or surgery.
