Enlarged thyroid: You need to find out why and what is causing your thyroid to get larger. A thyroid that is causing you to choke is very concerning. Make an appointment as soon as possible to see a doctor. This could be caused by lack of iodine to cancer and lots of things in-between. Do not wait, get it evaluated.

See ENT/endocrine: Enlarging thyroid nodules may necessitate biopsy or surgical removal of part or all of the thyroid. In patients with hypothyroidism, levothyroxine may help reduce nodule size. Otolaryngologist (ENT) and some general surgeons perform thyroidectomy.

Endocrine: Please see an Endocrine doctor, depending on what kind of lesion your thyroid have the treatment options are different, but if I take a guess you might have Multinodular Goiter which could be treated with Radioactive Iodine or surgery.
