Varicocele: there is no resolution for varicocele pain other than surgery. wearing more snug and comfortable underwear may help. avoid boxers

2 months ago, had hematospermia. US found small cyst on left epididymis & varicocele on left. No STD or UTI. PA gave abx, cleared pain & blood. Fine for month, then sore/achy in testes. Sore after ejac. Urologist says nothing wrong. Possible dx?
Varicocele never hurt but now after standing all day at work and randomly being active I get a ache and random pains. Is it time for surgery ?
