Low creatinine: Low creatinine in a child due to low muscle mass make the ratio looks higher. If there is no other problem with Renal Tubular Acidosis (RTA) or Uretero Pelvic Junction (UPJ) obstruction, this value is OK

Yes!: BUN is elevated, creatinine is normal, BUN:creatinine ratio is elevated indicates a Pre-renal kidney failure (not due to kidney itself). In pre-renal failure, the kidneys are not getting enough blood. In children, a BUN:Cr ratio of 30 or greater is highly indicative of upper gastrointestinal bleeding. Dehydration also considered. I assume his Nephrologist is aware.
