Poison ivy: If you had an exposure to poison ivy, you should wash yourself and all clothing/ items you wore at the time of exposure .Poison iv is an allergic reaction so sometimes the spread is due to worsening allergic reaction. If it is spreading you should seek medical attention as you may need oral steroids ( prednisone) to get the reaction under control. Calamine may be insufficient. Hope this helps

Contamination: The spread is from the resin on clothing and items contaminated by the poison ivy resin. You need to wash out everything and wipe clean any article including your shoes, car's floor mat etc. which might have come into contact with poison ivy. It is the resin which has caused the spread since poison ivy lesion is only localized to the area of contact. Try hydrocortisone cream in the meanwhile.
