What kind of lesion: You'll need to specify your "brain lesion" since this can mean many different types of problems from stroke, to tumor, to degenerative white matter disease, to demyelination or MS. The pain in the neck is more likely secondary to the herniated disc rather than an intracranial lesion.

Either or neither: Head and neck pain is very common and can be the result of a brain lesion or a result of a herniated disc. There is no way to determine the answer to your question-regardless of what type of brain lesion you have. Knowing which of the two choices is the cause of the symptoms also does not provide a predictable solution to the pain. Herniated discs are often not painful (of course they can be).
