You can't: Promethazine is only by prescription. It is potential side effects that could be dangerous like tardive dyskinesia, which is an irreversible movement disorder.

Promethazine: Your primary care provider would be able to prescribe promethazine if he/she felt it was appropriate to treat your vertigo type symptoms. During off hours, a provider within an urgent care center should be able to assess whether promethazine is appropriate for you. In the meantime, Cyclizine (Marezine), is similar to promethazine and is available OTC.
