Sounds like: post cholecystectomy syndrome. Other causes, such as retained stone, need to be excluded however. It could also be something unrelated such as zoster or pulmonary embolism. Consult with your doctor your surgeon to make sure.

Musculoskeletal : This is very unlikely related to a gall bladder operation from 18 months ago. It is important to know if this pain was present before surgery. Typically pain that is present to the touch is from something in the skin or right below. A rash would suggest shingles as would a history of chicken pox, recent illness, or recent stress. Irritation of the nerve that runs along the rib is possible as wel.
