Ilium or ileum?: This is a little confusing. The ilium is part of the bony structure of the pelvis. The ileum is part of the intestines. From the other part of your question I suspect you mean the ileum. If so then this pain could be caused from constipation. It could however be something more serious. I would try to relieve the constipation and if the pain does not resolve then I would seek medical care ASAP.
