Ear: Could be that the infected pimple has spread to that area of the ear. I would get this checked out as soon as possible. Go to an ER if need be

Likely Infected: It is difficult to tell without an exam or picture. However from your description it sounds as if you have an infection (small abscess) or infection in the mastoid region (mastoid air cells) This should definitely be looked at by your physician and possibly an ENT doctor. You likely need the infection drained and/or antibiotics.

The immune system is fighting germs to cause most of the inflammation. A collection of immune cells in an organized structure (lymph nodes) will cause a hard lump .. often behind or under the ear.
