Not sinusitis: There are at least 3 reasons for a perforated nasal septum. And sinusitis is definitely not one of them. Previous surgery for a deviated septum, trauma and cocaine abuse would be the most common causes. Who told you that sinusitis could be a cause of a hole in the nasal cartilage? That is not true.
