Apicoectomy is a procedure usually performed in a endo specialist office . In this procedure the end of the root tip is removed and a special cement or other filling material is placed to plug the freshly cut root tip.

Ways to save a tooth: When a root canal fails there are usually 2 options to save the tooth. Retreatment: the dentist basically redoes the rootcanal and hopes to do it better than the last one-i always recommend going to a specialist for this. Apicoectomy: removal of the tip of the root and sealing off of the tooth from the tip. This is a surgical procedure done if a retreatment isn't the right answer.

Ewhen possible non-surgical treatment is preferable. This is a redo of the first treatment. When not a choice a surgical approach is used to reach the cause of the infection. At times both are needed.

Different solutions: When a root canal treatment fails, it is usually because of an incomplete seal or accessory canals that are not sealed. If the problem cannot be resolved by retreatment of the original canal, a surgical approach is accessed at the root tip through the bone. The tip of the root or apex is removed and the canal is resealed from the root end which is an apicoectomy (removal of the apex).

Attempts to save: When a previously endodontically treated tooth fails, there are four basic options: 1-retreatment (rct redone, hopefully better) ;2-cut off the tip of the root & then seal it (surgical procedure); 3- retreat + apico; 4- extract the tooth when it is determined that likelihood of success is questionable & there are better long term solutions.
