Caseating granulomas: This sounds like tuberculosis, but may conceivably be systemic fungal infection like Histoplasmosis, rarely lymphoma or sarcoidosis. In India this is probably TB and either should be treated empirically or wait for the results of the biopsied lymph nodes. Sounds as if your doctor is very astute and would heed their advice.
