Anti coagulation: this is more a question of process of care to be addressed by hospital and outpatient quality process improvement committees. This would require a multidisciplinary approach to appropriate patient selection and patient monitoring.

Working through: the hospital's QM and Risk Management committees would be a good start. Collecting outcomes data, establishing goals and benchmarks, and a commitment to evidence-based medicine and standardized clinical pathways would be important.
