See answer: The primary indications for prostate biopsy to detect prostate cancer are either an abnormal DRE (nodule, induration/hardness, asymmetry) and/or an elevated PSA >2.5-4.0 (depending on what cutoff and guidelines one chooses to use). A PSA of 4.51 and a "hard" prostate in a 56 y/o male would be definite indications for a prostate biopsy with the risk of finding prostate cancer ~35-65%.
