GYN evaluation: You may have more than a UTI. Get a pelvic exam and kidney finction tests.. The nausea may be from Levaquin (levofloxacin). I suggest that you abstain sexually.

Urine Culture: Need a urine culture and sensitivity to make sure you on the right antibiotic. Next , if recurring UTI , may need Z Cystoscopy.
