Hard to say: Unfortunately this type of pain has a wide differential. Definitely, if it is severe or persists you should go the the closest emergency room. It could be a sign of something serious. Simple muscle or back strains can cause this type of pain. Shoulder pain can be a sign of heart disease or a gallbladder issue as well.

Spasm.: It sounds mostly muscular. Heat, massage, NSAIDS, rest and time heal most simple muscular injuries. See your doctor if painful or not improving in 5-7 days.
