Usually titrated up: Hi. Usually the dose of Qsymia (phentermine and topiramate) is titrated up, but I suppose if he tolerates the highest dose right off the bat and has a clinical response, it's OK. Talk with the doctor or speak with your pharmacist. Maybe the doc has had favorable experience starting right off at the highest dose.
