Concussion: Yes. The persistent symptoms require evaluation with an MRI of the brain. A concussion specialist may want to do neuropsychological testing to identify any subtle cognitive deficits. You will want to get cleared by your physician for sports and exercise too.

Possibly: Consult a neurologist or a neuro ophthalmologist. At the very least you should have a visual field test. You want to make sure you dont have an injury of the optic nerve or visual cortex in the brain.
