ER-now: Get an abdominal ultrasound and a HIDA scan. Suspicious for liver/gallbladder disease. Stop the Ultram.

No doctor is perfect: ...and it sounds like you have had some dissatisfactory interaction with your present one. Your symptoms are nonspecific, but warrant evaluation. I wouldn't hazard a guess here as to their cause or cure, but would suggest a second opinion by another GI doctor at your locale who can appropriately interview & examine you.
