Vasoconstriction: Cigarettes are associated with coronary artery vasoconstriction. This constriction could cause angina if combined with exercise in some people, but it would not do so in everyone. Much of the response would be based on the underlying health of the heart and coronary arteries.

Smoking does: increase the risk of an acute cardiac event, because it is a vasoconstrictor and it causes blood to be more coagulable, among other reasons. Because it increases the risk, doesn't mean it will happen all the time.
