Many Possiblities: Painful sex without injury does not usually lead to bleeding. There could be an injury that requires medical attention or it could simply be a coincidence with the onset of her cycle. At the very least - an exam is in order by a GYN doctor for both the pain and bleeding. These issues are treatable and preventable. Please take her to see a qualified GYN doctor ASAP.
