Depends: It depends on many factors. First off, you should know if you have no symptoms(like pain) or complications from the stone(like recurrent infections or blockage of the kidney) then it's unlikley that you need to have any procedure done. So if it's asymptomatic, I wouldn't encourage you to look for a procedure. But it also depends if the stone is in the "meat" of the kidney or the "pelvis".

Depends: 4.5 mm stone is at the upper limits of what may be passable through the ureter and into bladder. Usually < 5mm. Most urologists will give you a month trial to pass provided your kidney isn't swollen and functioning ok. He/she may also put you on something to help pass it. If becomes severely painful ESWL is the preferred choice if in kidney ( shock wave ). If moves, urologist may fetch.
