Can a small vericocele which is only felt when squeezed cause pain. is surgery necessary if the pain is mild.

No: Mild varicoceles that are not bothersome to the patient do not require surgical repair. However, the condition can cause pain, infertility, and testicular shrinkage in some more severe cases. In this case, discussion with the doctor is warranted with regard to surgical correction.

Here are some ...: The pain as described seems not related with varicocele itself so varicocelectomy is not needed in your case. Varicocel-related ache/pain usually occurs in an obvious large Varicocele usually after standing for a long duration so it happens in the afternoon/evning and improves/disappears in supine, lying position through overnight sleep.
