Do discharge reports from hospital show correct diagnosis or is it what patient is told when rounds done right before discharge? report also shows treatment with abx which was not done. cofused.

Of course: Your discharge papers show anything that was done or discussed during your hospital stay. You might not realize you had an antibiotic if it was done thru the IV during a procedure.
