constant pain under right shoulder blade. worst when laying on back and when lift arm. can it be gallbladder? no other symptom

Doubtful: not your classic gall bladder symptoms. Gall bladder issues really would not change or occur with movements of you arm. Sounds like a sprain. Get eval. best wishes.
