Classic Gilbert's: Gilbert's is an inherited syndrome. It is characterized by an elevated bilirubin. The inherited gene has a mutation in bilirubin metabolism, hence the elevated level. It can go undetected for a number of years, and is often found by accident. It is a common cause of an elevated bilirubin in an otherwise healthy individual. Fatty liver is also another common cause. Maybe, it depends.: There are many causes of elevated bilirubin, including Gilbert's syndrome and fatty liver. An initial work up, including liver function tests, complete blood count, hepatitis screening, and liver ultrasound, will provide a good starting point. Elevated bilirubin with all other tests being normal and no inciting medications is often due to Gilbert's.
