Carpal tunnel: See your surgeon to evaluate the hives to be sure that they are not related to the surgery. Infection or shingles can mimic hives so it is important to be evaluated.

Likely coincidental: The only thing I can think of would be a reaction to an ingredient in a long-acting cortisone if it was indeed injected along with the procedure. Even then, 4 weeks would is too long for that to continue. Are you taking any NSAIDs for post-op pain? If so, that may be the problem. Otherwise there is no relationship between the two events. Consult an allergist or dermatologist if this persists .
