Its unclear ...: I'm not sure what third level refers to. It sounds like your mother has degenerative changes, possible what's referred to as tricompartmental. Which means all three areas of the knee joint(outer, inner and behind the kneecap) are affected. Often knee replacement are recommended in this case. Again because I don't know your mother's specific it can't help more specifically. But this is a start
