May be neuropathy: This kind of pain is related to Neuropathy. Does he have a peripheral vascular disease or diabetes? These may be related. Please see a doctor.

Nerve sensitivity: He's probably feeling the shooting pain and stinging because the ulcer is becoming so deep that it's getting close to the nerves in that area and when touched or manipulated with dressing changes, it would cause a lot of pain like this. Plus, he might already have nerve sensitivity because of the diabetes.Are there signs of infection-warmth,redness,swelling?If so, may need antibiotics.
