Unusual: It is very unusual,just on the basis of COPD and asthma alone to have both atria to be enlarged unless there is something else like an arrhythmia like Atrial Fibrillation which is not uncommonly associated with chronic lung disease. Other causes like coexisting mitral valve disease would have been identified by the echo
