Exercise!!: Stretching, yoga, TaiChi, conditioning, physical therapy...... You get the idea. Only very hard work will get you better, but if you do it it will get you better. Maybe not to your prior baseline, but it will help a lot.

Post-stroke: Sounds like you have some degree of spasticity, which could be handled by medications or Botox. Physical therapy would be an excellent approach, and gradually develop an independent home exercise program. A neurologist or physiatrist can guide you.
