"Always"?: How long is "always"? The longer this has been true, without the generation of solid evidence of abnormality, the more probable it is due to stress. However, if it is new a brain stem lesion could produce it (would be a rare answer). CT scan of what? What result? MRI is better than CT for looking at brain structure when dealing with chronic problems.
