Yes thats a lot: Taking any short acting med up to 10 per day is inappropriate. I would suggest you speak with your brother about this and maybe he needs to be evaluated by another spine specialist to evaluate his needs. If not better he may need to be evaluated for the need for an opiate medication holiday & wean and restarted on the right medications, like a long acting med alongside a short acting med as well
