Odds are tiny, BUT: there are things you can get if the cigarette has enough of the smoker's saliva on it. For one thing, oral herpesvirus infections, which can cause everything from cold sores to encephalitis, can be spread in this way. If you have oral ulcers or mouth sores, it would even be possible, but highly unlikely, to pick up hepatitis B, and even more unlikely to pick up HIV or hep C.
