Could be, yes: Yes it could be broken. Obviously the first priority is to stop the bleeding. Direct pressure should help. If you are concerned about a fracture, an x ray will be needed.

Sure: Any time a heavy object drops on the foot/toe, there is the chance of a break, even if just a hairline fracture. Good luck to grandpa.
