Needs exam: I could write many paragraphs regarding possible long term complications of extensive abdominal surgery. I would also include urinalysis and prostate exam.

Painful urination: Two possibilities immediately come to mind. This may just be a urinary tract infection and a urinalysis is indicated. Second, he could have internal abdominal adhesions from the surgery which have attached to the bladder. Check the urine first at you primary care doc and go from there.
