How high?: A blood test will come back "high" for HCG in any woman who is pregnant. Anything positive gets coded as "high" by the lab.
Very high levels may be caused by multiple gestations (twins), abnormal pregnancy, or just be a variation of normal.

Yes.: Very high hcg. Levels can indicate an atypical pregnancy such a what is called a molar pregnancy. Strict follow up with your OB is important.

In short, yes: Dropping HCG levels mean an abnormal pregnancy if they are measured during the early weeks of pregnancy, this does not hold true if measured later in pregnancy when levels can drop and be normal. There are occasional time when they may not rise as expected when there are significant maternal illnesses as well.
