Not often: True vertigo is a symptom of spinning often associated with nausea. It is caused by inner ear disease, central (brain) disease, or even cervical arthritis. It is possible that active middle ear infection associated with a perforated ear drum can cause vertigo. If that is the case, that association may be very serious and needs the help of an ENT physician ASAP. Dry perforation not a problem.
