Splenomegaly: Pain is usually in left upper quadrant of the abdomen, sometimes into the thorax and shoulder by radiation from the diaphragm. It is conceivable to have the radiation pattern you describe but not probable. some of this may depend upon the cause of the splenic enlargement and whether it is inflammatory. This may need to be evaluated further with exam and imaging studies. Good luck.
